SummaryGuides patients through and around barriers in the complex healthcare system. Provides clinical coordination and therapeutic interventions through assessment, referral and interdisciplinary planning for high risk populations. The Behavioral Health Specialist will link patients and families to systems that provide resources, services and opportunities to achieve and maintain maximum functioning. The Behavioral Health Specialist empowers patients to be proactive with regard to their health and personal well-being.
